Instructions for Requesting a Summons in a Civil Case

 

If the filing fee for a non-prisoner civil case has been paid, or in forma pauperis status has been granted, please email your request for summons issuance by sending the summons in PDF format to the clerk’s office in the division where the case is pending. Remember to note the case number and complete all required fields on the form before emailing it to us. (If multiple summonses are needed, they may be included in one email.)

After the clerk confirms that the case is eligible for summons issuance, the clerk will email you the issued summons(es). You may then proceed to have them served under the provisions of Rule 4 of the Federal Rules of Civil Procedure.
